Editorial Comment: Comparative Cost-effectiveness of Surgery, Collagenase Clostridium Histolyticum, and Penile Traction Therapy in Men with Peyronie's Disease in an Era of Effective Clinical Treatment

CASE STUDY 40-year-old male patient and 32-year-old female partner, with a history of primary infertility of two years duration. The workup revealed idiopathic mild oligoasthenotheratozoospermia, and no apparent female infertility factors. The couple has failed three intrauterine insemination (IUI) cycles, planning more IUI cycles but also considering in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI).

Conclusions: This study provides the first evidence that a personality dimension with many prognostic implications may be altered in women with endometriosis. Our findings highlight the importance of a broader understanding of this condition, treating this disorder from a biopsychosocial perspective and suggests the need for non-medical attention within a multidisciplinary team. Abstract The mosaic 45,X/46,XY karyotype is a common sex chromosomal abnormality in infertile men. Males with this mosaic karyotype can benefit from assisted reproductive therapies, but the transmitted abnormalities contain 45,X aneuploidy as well as Y chromosome microdeletions. CONCLUSIONS: Recent work has identified associations between a number of malignancies and benign urologic conditions including male infertility, Peyronie's disease, cryptorchidism, and hypospadias. Molecular and genetic mechanisms have been proposed, but no definitive causal relationships have been identified to date. Future work will continue to better define the links between malignancy and benign urologic conditions and ultimately facilitate risk stratification, screening, and treatment of affected men. Plaque incision and grafting is an ideal technique to correct severe acquired penile curvature. Though there is no consensus on the best graft material, many options are available. The graft should allow for preservation of erectile function, resist infection, be watertight, have minimal contraction, and feel natural. Using an IRB-approved database, we present our contemporary experience of 93 patients with Peyronie's curvature treated with plaque incision and grafting using a temporalis fascia free graft.

Grafting techniques are preferred for the surgical treatment in advanced Peyronie ′s disease (PD) to avoid penile shortening. In this video we present the novel suture-free Sealing technique for Peyronie′s repair using a self-adhesive collagen fleece as an alternative to grafting. We moreover report on long-term surgical outcomes of this novel technique.
