Are specialized sperm function tests clinically useful in planning assisted reproductive technology?

CASE STUDY 40-year-old male patient and 32-year-old female partner, with a history of primary infertility of two years duration. The workup revealed idiopathic mild oligoasthenotheratozoospermia, and no apparent female infertility factors. The couple has failed three intrauterine insemination (IUI) cycles, planning more IUI cycles but also considering in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI).

Abstract The mosaic 45,X/46,XY karyotype is a common sex chromosomal abnormality in infertile men. Males with this mosaic karyotype can benefit from assisted reproductive therapies, but the transmitted abnormalities contain 45,X aneuploidy as well as Y chromosome microdeletions. We have known for over 40 years that in the setting of in  vitro fertilization (IVF), having poor semen parameters will often lead to poor fertilization rates, resulting in suboptimal reproductive outcomes) (1). Overcoming this “male factor” infertility with the use of intracytoplasmic sperm injection (ICSI) has arguably been among the biggest revolut ions in IVF practice since its first use in 1992. As pointed out by Sunderam et al. ConclusionsThe rate of successful TESE and the ICSI outcome in cancer survivors with NOA and RE/FOE is the same as non-cancer azoospermic patients. Female partner age (older than 40  years) was associated with a significant reduction in live birth rates after TESE-ICSI procedures.
