Same results, 20% of the cost: Short-course total neoadjuvant therapy

We would offer this patient short-course radiation (25 Gy/5 daily fractions), followed by consolidation mFOLFOX (known as short-course total neoadjuvant therapy) for quicker, cheaper, less toxic, and more effective downstaging. Radiation would be delivered with standard pelvic volumes, except for lowering the superior border to S2/S3. Intensity modulated radiation therapy would only be used if small bowel exposure would exceed 25 Gy. After a 2-week break to allow resolution of radiation-induced proctitis, mFOLFOX is administered in standard fashion before tumor restaging.
