Conclusions: SMART provided promising oncologic outcomes at the cost of significant treatment related morbidity. Due to the significant treatment associated morbidity and favorable treatment alternatives, we have not broadly adopted SMART at our institution.

Abstract BACKGROUND: Ethnicity may be associated with important aspects of end-of-life care, such as what treatments are received, access to palliative care and where people die. However, most studies have focused on end-of-life care of white, Hispanic and black patients. We sought to compare end-of-life care delivered to people of Chinese and South Asian ethnicity with that delivered to others from the general population, in Ontario, Canada. METHODS: In this population-based cohort study, we included all people who died in Ontario, Canada, between Apr. 1, 2004, and Mar. 31, 2015. People were identified as ha...

Children with chronic illnesses have complex health-care needs, leading to frequent health-care use for acute illness, high medical care costs, and ultimately, poor health outcomes. Children with chronic illness and medical complexity (CIMC) are patients under the age of 18 who have three or more of the following: (1) medications and/or supportive therapies (nutrition, respiratory, dialysis); (2) chronic medical diagnoses for 12 months or more; and (3) dependence on technology (central or long-term peripheral line, tracheostomy, gastrostomy; Cohen et al., 2011).

ConclusionsEighteen factors may increase the risk of carbapenem resistance inK. pneumoniae infection; eight factors may be associated with bothK. pneumoniae infections in general and CRKP in particular. The eight shared factors are likely to be ‘true’ risk factors for CRKP infection. Evaluation of risk factors in different situations may be helpful for empirical treatment and prevention of CRKP infections.

CONCLUSION: Severe bacterial soft tissue infections can be still life-threatening despite guideline-conform treatment and are associated with a high mortality especially in high-risk patients even with intensive medical care. They can lead to disfiguration and loss of function resulting in a greatly reduced quality of life. Rapid diagnostics and therapy are essential for successful treatment. AbstractBackgroundContemporary data on mitral valve (MV) surgery in patients with infective endocarditis (IE) are limited.MethodsThe National Inpatient Sample was queried to identify patients with IE who underwent MV surgery between 2003 and 2016. We assessed (a) temporal trends in the incidence of MV surgery for IE, (b) morbidity, mortality, and cost of MV repair vs replacement, and (c) predictors of in ÔÇÉhospital mortality.ResultsThe proportion of MV operations involving patients with IE increased from 5.4% in 2003 to 7.3%, and the proportion of MV repair among those undergoing surgery for IE increased from 15.2% to 25.0% (Ptrend

AbstractThis is a single-center retrospective study to summarize clinical outcomes of patients requiring surgical continuous-flow left ventricular assist device (HeartMate II) exchange. The patients who underwent HeartMate II exchange were divided into two groups either via a subcostal approach (SC group) or a full sternotomy (FS group). The exclusion criteria of a subcostal approach for device exchange included the presence of outflow graft obstruction, and/or the need for concomitant cardiac procedures. Among 277 consecutive patients who underwent HeartMate II implantation from July 2008 to December 2015, 25 patients (9....

AbstractBackgroundData regarding who will require tracheostomy are lacking which may limit investigations into therapeutic effects of early tracheostomy.MethodsWe performed an observational study of adult traumatic brain injury (TBI) patients requiring intensive care unit (ICU) admission for  ≥ 72 h and mechanical ventilation for ≥ 24 h between January 2014 and December 2014 at a level 1 trauma center. Patients who had life-sustaining measures withdrawn were excluded. Multivariable logistic regression analyses were used to assess admission and inpatient factors associated with re...
