Considering the proportional mass of this area, the foot and ankle are affected more frequently by neoplasia than the rest of the musculoskeletal system. Nevertheless, correct diagnosis is often delayed, and unplanned resection of soft tissue sarcomas is not uncommon. Due to the rarity and heterogeneity of this pathology, even experienced orthopaedic surgeons can fail to initiate the correct steps in diagnostics and treatment, potentially negatively affecting the outcome. This case report demonstrates the prolonged course towards the correct diagnosis of a pleomorphic leiomyosarcoma of the ankle. Interdisciplinary treatment consisted of an open biopsy followed by wide resection and plastic surgical reconstruction including a gracilis muscle flap, a LFTA reconstruction with the harvested gracilis-tendon and adjuvant radiotherapy.ZusammenfassungUnklare Raumforderungen im Bereich von Fuß und Sprunggelenk sind häufig, aber echte Neoplasien des Bewegungsapparates sind selten und Weichteilsarkome machen nur 1% aller Malignome aus.
