We asked whether changes in noninvasive markers of fibrosis can predict liver-related complications.MethodsThis was a case-controlled study using a prospective national cohort (ANRS-CO12-CIRVIR) of 1323 HCV-infected patients with compensated cirrhosis: 97 patients who developed liver-related complications such as hepatocellular carcinoma or hepatic decompensation (cases) matched in age, sex and follow-up duration were compared with 257 patients without complications (controls). Actitest/Fibrotest™, Inflameter/Fibrometer™, ELF™ and Fibroscan™ were performed at baseline and yearly. Samples based on Propensity score matching were built and mixed linear models performed. ConclusionLaparoscopic caudate lobectomy for cirrhotic patients with partial IVC resection is technically demanding. It therefore requires a strategic and preplanned approach with dedicated instrumentation and laparoscopic skills available. Although the caudal view along the axis of the IVC facilitates dissection, a laparoscopic approach necessitates particular attention to central venous pressure management (intravenous fluid and respiratory tidal volume), meticulous control of portal and short hepatic vein branches, and availability of specialty laparoscopic instrumentation to ensure procedural safety.

A 63-year-old man with complaint of painful oral ulcerations with 10 days of evolution was referred to the service. Medical history revealed a recent liver transplant owing to a severe liver dysfunction caused by a hepatocellular carcinoma and cirrhosis caused by hepatitis C virus and end-stage renal disease. Intraorally, multiple painful ulcerative lesions associated with white plates were observed. Investigation revealed increased levels of urea. The diagnosis of uremic stomatitis was established.
