Use of the Keystone Perforator Island Flap in the Treatment of Chronic Lower Extremity Wounds Complicated by Osteomyelitis

Soft tissue reconstruction of chronic lower extremity wounds represents a challenging problem, especially when wounds are complicated by bone infection. Treatment consists of multi-disciplinary management with plastic and orthopaedic surgeons and the support of the infectious disease team. Surgery involves radical debridement and soft-tissue coverage preferably carried out at the same time, as well as appropriate antibiotics guided by the microbiology results from the bone specimen. Normal bone is highly resistant to infection with a low overall incidence of osteomyelitis [1], in the US osteomyelitis arises in 22 per 100,000 people each year [2].
