Intraoperative Diagnosis of Detached Necrotic Fibroid

A 58-year-old woman with enlarging fibroid uterus in menopause presented for gynecologic evaluation. The enlarging, pedunculated, exophytic fibroid measuring 8.7  × 10 × 10 cm was noted incidentally on CT performed for an episode of acute abdominal pain attributed to diverticulitis. Three years earlier, the fibroid measured 4.0 × 4.2 × 3.9 cm on transvaginal ultrasound. Given the concern for an enlarging fibroid in menopause, MRI was perf ormed which showed a pedunculated fundal fibroid measuring 8.5 × 7.2 × 9.2 cm and progressively enhancing right ovarian cyst measuring 3.5 × 2 cm (Figure 1).

In this study, benzyl benzoate was successfully synthesized via enzymatic acylation using three immobilized enzymes as biocatalysts. Different acyl donors (benzoic acid and benzoic anhydride), operation regimes (batch, fed-batch), mixing modes (conventional mechanical stirring and ultrasound), process parameters (temperature, substrate molar ratio of acyl donor to acyl acceptor), presence or absence of solvents, enzyme amount and type were evaluated. Benzoic acid is a solid that is difficult to solubilize and, thus, was not efficient as acyl donor for the synthesis of benzyl benzoate. On the other hand, benzoic anhydride w...

This week's very cool case was donated by Dr. Mike Feely. We haven't had anything like this one before on the blog! The specimen below was submitted by a young woman who found this " worm " by her arm, right below her mouth, upon waking from a nap. She reported having abdominal pain with associated diarrhea for several weeks prior to presentation, but was otherwise healthy. After being in formalin:Histologic sections (H&E):Identification? Any additional information that you would like?

CONCLUSION: MRA failed to reveal abnormalities in patients with a clinical diagnosis of VBI compared with controls. The PI of the basilar artery, measured using TCD, demonstrated high sensitivity (91%) and specificity (91%) for detecting clinically diagnosed VBI.

OBJECTIVES: Alcohol for intercostal neuralgia may induce severe injection pain. Although nerve block provided partial pain relief, alcohol might be diluted, and the curative effect decreased when the local anesthetic and alcohol were given at the same point. Therefore, we observed the modified method for intercostal neuralgia, a Two-point method, in which the local anesthetic and alcohol were given at different sites. METHOD: Thirty patients diagnosed with intercostal neuralgia were divided into 2 groups: Single-point group and Two-point group. In the Single-point group, alcohol and local anesthetic were injected at the sa...

AbstractIntroductionWe compared the preoperative quality of life (QOL) of patients with carpal tunnel syndrome, lower extremity-, and para-lumbar entrapment syndrome, and the effect of surgery on their QOL.Patients and methodsWe prospectively enrolled 66 consecutive patients who underwent surgery for carpal tunnel syndrome (group 1,n = 23), lower extremity entrapment syndrome (group 2,n = 22), and para-lumbar entrapment syndrome (group 3,n = 21). Their pre- and postoperative overall health status was assessed on the Medical Outcomes Study Short-Form 36 Health Survey, v2 (SF-36).Res...
