Neurological Complications in Patients with Systemic Lupus Erythematosus

AbstractPurpose of ReviewSystemic lupus erythematosus (SLE) is commonly associated with neurological manifestations. Rapid recognition and treatment of these complications may improve outcomes. In this article, we review the neurological conditions associated with SLE, their diagnosis and management strategies.Recent FindingsRecent meta-analysis showed that patients with neuropsychiatric manifestations of SLE were more likely to have positive antiphospholipid, antiribosomal P, and antineuronal antibodies. Another meta-analysis showed an association between SLE and antiphospholipid antibodies with cognitive impairment. Two large retrospective studies have shown that the peripheral nervous system is commonly involved in SLE frequently alongside the central nervous system.SummaryNeurological manifestations occur in most of SLE patients. Antiphospholipid antibodies are common in patients with SLE and increase the odds of neurological complications. Management typically involved a combination of treatments directed toward the neurological complication and therapies directed toward SLE itself. The efficacy of these treatment protocols, however, has not been rigorously studied and deserves further investigation.

This study characterized trends in utilization of CABG in patients with CTD and the impact of the disease on mortality, in-hospital complications, length of stay (LOS) and costs. Its major presentations are thrombotic (arterial, venous or microvascular) and pregnancy morbidity (miscarriages, late intrauterine fetal demise, and severe pre-eclampsia). Classification criteria include three different antiphospholipid antibodies: lupus anticoagulant; anticardiolipin; and anti-beta 2 glycoprotein I. CONCLUSIONS: D-S limits DOAC interference in LAC assays. DOAC concentration measurement should be performed in D-S treated samples as incomplete removal may occur. Applying D-S to VKA-containing, heparin-containing or not-anticoagulated samples may lead to erroneous LAC results. Therefore, D-S should only be used in plasma from DOAC-treated patients.
