Hormonal therapy (Tomaxifen) is the frontline treatment for hormone-dependent breast cancers .Progesterone hormone induces its action on the target cells by binding with its Progesterone receptor (PgR) therefore any genetic variations, which might induce alienation in the progesterone receptor, can result to an increased susceptibility to gynecological cancers. Alu insertion (PROGINS) mutation in PgR gene is reported to be associated with an increased risk of ovarian cancer and a decreased risk of breast cancer. Hormone therapy is an effective and non-toxic therapy for oestrogen and progesterone receptor-positive breast cancer and for prostate cancer. Serum concentrations of oestradiol and testosterone are controlled by the hypothalamic-pituitary-gonadal pathway. Oestradiol is produced in premenopausal women from the ovaries, and in postmenopausal women by peripheral conversion of adrenal androgens by aromatase. In premenopausal women with breast cancer and men with prostate cancer, treatment is primarily achieved by castration.

Conclusion: There does not exist an optimal treatment sequence for hormone-receptor-positive, her2-negative advanced breast cancer. However, after review of literature, a reasonable approach may be starting with tamoxifen, aromatase inhibitors or fulvestrant in absence of visceral crisis, in addition to ensuring adequate ovarian function suppression in pre/peri-menopausal women.
