A Novel Technique for Thoracic Duct Access through MR Imaging/Ultrasound Fusion: Successful Percutaneous Embolization of Pulmonary Lymphatic Vessels

Plastic bronchitis is a potentially fatal disorder characterized by the exudation of proteinaceous material and cells in the airways that leads to branching cast formation, which can be expectorated or lead to asphyxiation. Plastic bronchitis can appear as a complication after single-ventricle palliation. Other causes of plastic bronchitis that have been described include cystic fibrosis, sickle-cell anemia, asthma, and lymphangiomatosis. The underlying pathophysiology of plastic bronchitis seems to be abnormal pulmonary lymphatic perfusion, and the preferred treatment is embolization of the abnormal pulmonary lymphatic vessels (1).

We report a case in which diffusion-weighted magnetic resonance imaging (DWI) demonstrated renal artery stenosis-related renal ischemia and the therapeutic efficacy of revascularization. A 52-year-old farmer was admitted with recurrent fever for>1 year, hematuria, proteinuria, renal insufficiency, and anemia for 4 months. He lost 15 kg over 1 year. He was employed in the animal husbandry industry including breeding of cattle and sheep and slaughtering cattle. On admission, the patient was afebrile and his blood pressure was 110/70 mm  Hg. Physical examination revealed 3 enlarged lymph nodes bilaterally in the inguinal area and was otherwise normal.
