Diagnostic assessment of patients with Meniere's disease through caloric testing and the video-head-impulse test

Publication date: Available online 22 November 2019Source: Brazilian Journal of OtorhinolaryngologyAuthor(s): Lívia Noleto de Rezende Oliveira, Cleydson Lucena de Andrada Oliveira, Karen de Carvalho Lopes, Fernando Freitas GanançaAbstractIntroductionMeniere's disease is a labyrinth disease that usually presents with episodes of spontaneous vertigo associated with sensorineural hearing loss, tinnitus and ipsi- and unilateral aural fullness in most cases. Vestibular function tests, video-Head-Impulse Test and the Caloric Test, are not specific for diagnosis of the disease, but may show alterations that help to evaluate the functional impairment.ObjectiveTo describe the results obtained at the Caloric Test and video-Head-Impulse Test in patients with definite Meniere's disease and compare them between symptomatic, asymptomatic ears and those of the control group.MethodsCross-sectional and observational study including patients with definite Meniere's disease diagnosed according to the Bárány Society criteria (2015) and healthy individuals (control group) undergoing Caloric Test and video-Head-Impulse Test. All subjects were assessed by neurotological anamnesis and audiological evaluation (pure-tone, vocal and immittance audiometry) to characterize the sample. The findings obtained at the Caloric Test and video-Head-Impulse Test were described and compared between the symptomatic and asymptomatic ears of patients with Meniere's disease and those of the...

What is tinnitus? Tinnitus is a generic term used to describe a ringing or noise in the ears that occurs in the absence of external sound. This is a very common condition that is thought to occur in up to 15% of people. It can occur in one or both ears, and often people will describe the sound as “coming from their head.” There are a variety of descriptions that people use for their tinnitus such as whooshing, ringing, pulsing, and/or buzzing, and the quality of the sound varies by individual. Symptoms of tinnitus can cause great distress While tinnitus can be caused by conditions that require medical attention...

Previous studies have demonstrated the role of blood pressure in Meniere's disease. The purpose of this study was to determine the role of ACE, rs4646994 I/D polymorphism in Meniere's disease. Methods ...

DiscussionOur planned systematic review will provide informative evaluations of existing treatments for management of Meniere ’s disease. The findings will inform practitioners as to the relative benefits and harms of the existing competing interventions for MD, offer optimal clinical treatment strategies, identify evidence gaps, and determine promising therapies for evaluation in future trials.Systematic review registrationPROSPEROCRD42019119129

Semin Neurol DOI: 10.1055/s-0039-3402065Meniere's disease (MD) is a set of uncommon disorders with core phenotype of tinnitus, episodic vertigo, and sensorineural hearing loss. MD shows a genetic predisposition and a family history is found in 10% cases, with an autosomal dominant inheritance pattern. It is a multifactorial condition whose onset and development are triggered by the combined effect of genetic and environmental factors. Histopathological studies have associated MD with the accumulation of endolymph in the cochlea and the vestibular organs. However, endolymphatic hydrops does not fully explain the persistence...

ConclusionOur study shows a reduced number of vertigo attacks and a positive effect on the discomfort generated by tinnitus and quality of life in patients with unilateral MD treated with SPC and when compared to patients treated with intravenous glycerol and dexamethasone. No effects on hearing thresholds were noted in both groups.

RauchMeniere's disease is characterized by idiopathic progressive fluctuating hearing loss, tinnitus, and episodic vertigo. Intratympanic gentamicin injection is the preferred therapeutic option for those who have failed conservative management including lifestyle modifications and oral medications. The procedure is simple and can be performed in the outpatient setting under local anesthesia with manageable recovery time. Patients may experience a disequilibrium s...

Two cases of unilateral SNHL who were unresponsive to conventional treatment of sudden SNHL showed complete recovery after receiving EA therapy. The first case was a 46-year-old female who received EA in the seventh month after the acute onset of sudden right hearing loss and tinnitus. She ha...

In conclusion, for the radiological assessment of TB fractures, the entire VA should be regarded as a part of the otic capsule, and delayed inner ear sequelae should be anticipated for retrolabyrinthine fracture lines that course into or through the VA. When considering treatment options in cases similar to the present, our findings suggest that surgical interventions targeting the eES (ES shunting/decompression procedures (31–33), which are used in MD with the intention to drain the hydropic endolymphatic fluid space, or to improve the fluid resorptive functions of the eES, respectively, most likely cannot work as p...

Many anatomical studies have suggested idiopathic endolymphatic hydrops as the pathological basis of Meniere's disease, which now can be visualized by using gadolinium -enhanced magnetic resonance imaging of the inner ear.ObjectiveTo investigate the development of endolymphatic hydrops in Meniere's disease by monitoring the vestibules and cochleae of af...
