Thyroid Nodules and Cancer during Pregnancy, Post-partum and Preconception Planning: Addressing the Uncertainties and Challenges

Publication date: Available online 22 November 2019Source: Best Practice &Research Clinical Endocrinology &MetabolismAuthor(s): Maria Papaleontiou, Megan R. HaymartAbstractThyroid nodules and thyroid cancer have become increasingly common worldwide. When discovered during pregnancy, they pose unique diagnostic and therapeutic challenges for both the treating physician and the patient. The benefits of treatment should be carefully weighed against risks that may adversely impact maternal and fetal health. In this review, we present current knowledge and controversies surrounding the management of thyroid nodules and thyroid cancer in pregnancy, in the post-partum period and during preconception planning.

Discussion Tachycardia is a rapid heart rate that is above normal for age and level of exertion. Tachycardia is common, particularly sinus tachycardia due to normally encountered circumstances such as pain, fever or exercise. It is usually a normal physiologic process but sustained tachycardia often indicates a potentially abnormal underlying cause. Sinus tachycardia has a rapid heart rate with normal P waves and P-R intervals and variations from moment to moment and respiration. Generally it is not over 200 beats/minute. Vagal stimulation can slow the heart rate; this is a gradual slowing, not an abrupt slowing seen in ...

This study aimed to evaluate fertility in female survivors of childhood DTC by assessing various reproductive characteristics combined with anti-Müllerian hormone (AMH) levels (a marker of ovarian reserve). Methods. Female survivors of childhood DTC, diagnosed at ≤18 years of age between 1970 and 2013 were included. Survivors were excluded when follow-up time was less than 5 years or if they developed other malignancies before or after diagnosis of DTC. Survivors filled out a questionnaire regarding reproductive characteristics (e.g. age at menarche and menopause, pregnancies, pregnancy outcomes, need for assisted ...

In this study, a highly sensitive and accurate LC–MS/MS method was firstly established for subsequent measurement of PdG in serum of three different female groups: thyroid cancer patients (TCs), healthy controls (HCs) and pregnant women. The factors affecting the sample preparation, MS/MS method, gradient elution program, selection of chromatographic column and internal standard (IS) have been optimized in this study. Compared with enzyme immunoassay (EIA) method, we used LC–MS/MS to shorten analysis time, increase sensitivity, raise specificity, simplify sample preparation, and reduce costs. As a result, the l...

Often we don’t really consider gender dynamics in treatment or medication. A lot of medications are only tested on men because of the risk of pregnancy, etc. This means there are whole drugs that have made it to market that may not have ever been tested with women. Schizophrenia affects women in many different ways than men. In this episode schizophrenic Rachel Star Withers and cohost Gabe Howard discuss differences in age, symptoms, treatments, lifestyle, parenthood in the genders as they experience schizophrenia. Dr. Hayden Finch joins to explain the medical side.  Highlights in “Schizophrenia in Women...

Thyroid radiation doses were estimated for a cohort of 2,965 Belarusian persons who were exposed in utero and during early life to fallout from the Chernobyl nuclear power plant accident. Prenatal and postnatal doses to the thyroid due to intake of 131I, external irradiation from radionuclides deposited on the ground, and ingestion of cesium isotopes (134Cs and 137Cs) were calculated for all cohort members. Dose estimation was based on personal interviews with subjects’ mothers; the interviews collected data on subjects’ residential history, consumption by mothers during time of pregnancy and breast-feeding, as...

CONCLUSIONS: This article presents the first detailed information on several key aspects of daily life on French Polynesian archipelagoes during the 1960s-1970s impacting radiation exposure. Important behavior and food consumptions data obtained in this study are being used to improve the radiation dose estimates and to update the risk analysis reported earlier by correcting biases from previous assumptions and by providing better estimates of the parameter values important to radiation dose assessment.
