Diagnostic value of bronchoscopy for peripheral metastatic lung tumors

When we encounter cases with peripheral pulmonary lesions (PPLs) through cancer therapies, the pathological diagnosis is crucial to determine an accurate treatment plan. However, diagnostic bronchoscopy for metastatic lung tumors tends to have a difficult impression. Thus, we investigated the diagnostic value of bronchoscopy in this situation.Consecutive cases who underwent diagnostic bronchoscopy with radial endobronchial ultrasound (R-EBUS) for PPLs from April 2012 to March 2017 were collected. Among them, the cases finally diagnosed as metastatic lung tumor were enrolled. The diagnostic outcome and the factors affecting it were analyzed; including factors were age, sex, lesion size/lobe/location, bronchus sign, distance from costal pleura, visibility on X-ray and use of navigation/guide sheath/needle.We analyzed 179 cases; the median (range) lesion size was 19.5 (6.0-80.8) mm, and 75 cases showed negative bronchus sign. The overall diagnostic yield was 71.5%; there was a significant difference depend on EBUS images (within vs. adjacent to vs. invisible: 92.3% vs. 67.9% vs. 17.4%, p20.0 mm vs.
