Efficacy of using special software "Optimization of medication choice for treatment of patient with comorbidity" (SS) for management of patients with community-acquired pneumonia (CAP) and cardiovascular disease

Treatment of CAP at the patient with cardiovascular diseases is difficult task because except effectiveness of antibiotics risk of adverse events and drug interaction between basic cardiovascular medicines and antibiotics should be taken into account. We have created SS. Using it doctor enters complete clinical diagnosis of patient and receives recommended by guidelines list of medications for therapy with consideration of their benefit and dangerous interactions. We compared efficacy and safety of ordinarily prescribed treatment of 157 CAP patients with cardiovascular diseases (control group - CG) and using SS prescribed treatment of 135 same patients (basic group-BG). Average number of prescribed medicines in CG was 11.8±4.2 but in BG – 8.9±2.5 (p

Pericardial effusion is often detected in patients with Infective endocarditis. Incidence can be as high as 25% . Most often it is mild, can be moderate in few. Mechanism Sympathetic effusion in response to endocardial infection. It’s never more than minimal. (Evidence ? it’s only an assumption) IE related cardiac failure (Raised systemic venous pressure to which pericardial veins drain) Local sepsis, Abcess formation tracks to pericardial space through transmural lymphatics Fungal , granulomatous , Tuberculous IE (Rare) Here IE and PE  share the same pathology Part of systemic sepsis activated Immun...

As researchers fight to stop the spread of a novel coronavirus that originated in a seafood market in the central Chinese city of Wuhan, they are facing a counter-intuitive problem: the symptoms can sometimes be so mild in patients that the virus is very difficult to detect. The Wuhan coronavirus, dubbed 2019-nCoV, has killed at least 106 people in mainland China, out of the 4,400-plus documented cases. According to records released by Chinese health authorities, most fatalities were older or had additional health problems. Severe acute respiratory syndrome (SARS), which is related to 2019-nCoV and killed nearly 800 people...

Influenza A virus (IAV) is among the most common causes of pneumonia-related death worldwide. Pulmonary epithelial cells are the primary target for viral infection and replication and respond by releasing inflammatory mediators that recruit immune cells to mount the host response. Severe lung injury and death during IAV infection result from an exuberant host inflammatory response. The linear ubiquitin assembly complex (LUBAC), composed of SHARPIN, HOIL-1L, and HOIP, is a critical regulator of NF-κB–dependent inflammation. Using mice with lung epithelial–specific deletions of HOIL-1L or HOIP in a model of...

CONCLUSIONS: In people with mild GERD, on-demand deprescribing may lead to an increase in GI symptoms (e.g. dyspepsia, regurgitation) and probably a reduction in pill burden. There was a decline in participant satisfaction, although heterogeneity was high. There were insufficient data to make a conclusion regarding long-term benefits and harms of PPI discontinuation, although two trials (one on-demand trial and one abrupt discontinuation trial) reported endoscopic findings in their intervention groups at study end.
