Pancreatic Fiducial Markers Placement: Time Is On My Side

We read with great interest the article by Machicado et  al,1 which reported the results of a randomized controlled trial comparing the performance of pre-loaded fiducial needles (PLNs) versus traditional back-loaded fiducials (BLNs) in patients with pancreatic ductal adenocarcinoma (PDAC) undergoing image-guided radiation therapy (IGRT) in 2 academic t ertiary care centers. Among 44 patients enrolled into the study during a 22-month period, 22 underwent PNLs using a novel fiducial needle with 4 gold markers (PreLoad4; Cook Medical, Bloomington, IN), whereas in other 22 patients assigned to the BLN group, the Visicoil fiducials (Core Oncology, Sant a Barbara, CA) were used.
