Retiring, Changing Specialty to lower risk/pay or part time work if you had 11 Million in assets

I am an MD/MBA (obtain MBA later at top institution while working) whereby I specialized in Anesthesiology. I have a trust that I will be inheriting with about 8M where I am worth approximately 3M, so the total assets come out to be 11M. Assets are most liquid in a mix of stock index funds and bonds, with about 50K in misc Gold holding and 425K in a house that I bought with cash. When I run the NPV numbers at 7% (assumption for long term returns) for 10.5M in assets (liquid portion), I... This study is an attempt to investigate the effects of intrathecal administration of Melissa officinalis on the pain induced by heat and formalin. In this experimental study, 70 male Wistar rats with an average weight of 270–320 g were randomly divided into five groups: control; sham that received 25 μl of saline through the spinal catheter; and three experimental groups that received 5, 10 or 20 mg/kg M. officinalis via the spinal catheter respectively. Five days after catheterization of the spinal cord from the lumbar region under anesthesia, the effects of Intrathecal administration of M...

BACKGROUND Postictal delirium (PID) is a relatively common complication following electroconvulsive therapy (ECT). OBJECTIVE We investigated whether prophylactic dexmedetomidine administration would safely decrease the incidence of PID in psychiatric patients after ECT. DESIGN A randomised, double-blind, placebo-controlled trial. PATIENTS A total of 223 patients undergoing ECT were randomly allocated to two groups. INTERVENTIONS Patients received 0.5 μg kg−1 dexmedetomidine (Dex group, n=111) or 0.9% sodium chloride (Con group, n=112) before ECT. Propofol was used for anaesthesia and succinylchol...

CONCLUSION POD is prevalent in patients after elective intracranial surgery. The identified risk factors for and the potential association of POD with adverse outcomes suggest that a comprehensive strategy involving screening for predisposing factors and early prevention of modifiable factors should be established in this population. TRIAL REGISTRATION ClinicalTrials.gov NCT03087838.

The objective of this study was to compare the initial intubation in the cardiac operating room with reintubation (if required) in the ICU following cardiac surgery. DESIGN A prospective, observational study. SETTING Department of Anesthesiology and Intensive Care Medicine, Clinical Hospital of Santiago, Spain. PATIENTS With approval of the local ethics committee, over a 44-month period, we prospectively enrolled all cardiac surgical patients who were intubated in the operating room using direct laryngoscopy, and who required reintubation later in the ICU. MAIN OUTCOME MEASURES The primary endpoint was to compa...

BACKGROUND There is inadequate information on the association of pre-operative and postoperative peak level of lactate with mortality of surgical ICU patients. OBJECTIVE To investigate the association between peri-operative lactate level and 90-day mortality in patients admtted to the surgical ICU. DESIGN Retrospective cohort study. SETTING ICUs in single tertiary academic hospital. PATIENTS Adult patients postoperatively admitted to the ICU between January 2012 and December 2017. INTERVENTION None. MAIN OUTCOME MEASURES Hazard ratios of 90-day mortality according to the following serum lactate levels w...

BACKGROUND Neuromuscular blockade (NMB) monitoring is essential to avoid residual NMB. While the adductor pollicis is the recommended site for monitoring recovery, it is not always accessible. The flexor hallucis brevis could be an interesting alternative. OBJECTIVE The aim of our study was to compare NMB onset and recovery at both sites. DESIGN Prospective observational study. SETTING Operating rooms at the University Hospital of Poitiers, France. PATIENTS Sixty patients scheduled for surgery under general anaesthesia with neuromuscular blocking agents were enrolled from January 2016 to September 2017. Data ...

BACKGROUND Postoperative residual curarisation (PORC) is a risk directly related to the use of neuromuscular blocking agents during surgical procedures. Acceleromyography is distressing for conscious patients when assessing PORC. Diaphragm ultrasonography could be a valid alternative. OBJECTIVES The primary objective was to achieve a 28% lower incidence of PORC in patients who, after rocuronium administration, received neostigmine or sugammadex at 30 min after surgery. To assess PORC, diaphragm ultrasonography was used, and thickening fractioning [the difference of thickness at the end of inspiration (TEI) and at th...
