Application of Monochromatic Imaging and Metal Artifact Reduction Software in Computed Tomography Angiography after Treatment of Cerebral Aneurysms

This study aimed to evaluate the image quality and degree of metal artifact reduction using the new-generation gemstone spectral imaging (GSI) and metal artifact reduction software (MARs) and to demonstrate the optimal monochromatic energy level for dual-energy cerebral computed tomography angiography (CTA) in patients with intracranial aneurysm after endovascular treatment. Material and Methods A total of 20 patients with cerebral aneurysms treated with coils or clips underwent CTA using gemstone spectral computed tomography. Artifact index was calculated at each energy level with and without MARs. Signal-to-noise (SNR) and contrast-to-noise (CNR) ratios were calculated on all axial images with MARs; subjective evaluation was done by using a 4-point scale and a 3-point scale for assessing noise and vessel contrast, respectively, and compared between the monochromatic energy levels. An 11-year-old boy with Klippel-Trenaunay syndrome with right lower extremity and pelvic venous ectasia underwent prophylactic embolization of persistent sciatic vein and markedly ectatic internal iliac vein using 24 coils (20 mm x 20 cm to 12 mm x 14 cm) (Nester; Cook Inc, Bloomington, Indiana) (Fig 1) to reduce risk of pulmonary embolism. He presented to the emergency department 3 days later with mild chest discomfort. This report identifies a rare complication of transforaminal full-endoscopic surgery in the treatment of a herniated lumbar disc. To our knowledge this is the first case of pseudoaneurysm formation of the lumbar artery following a full-endoscopic transforaminal lumbar discectomy.

A 78-year-old man underwent endovascular abdominal aortic aneurysm repair (EVAR) for a true aortic aneurysm. He developed sudden abdominal pain four hours after EVAR. Angiography revealed a dissected aneurysm of the middle colic artery (MCA). Despite conservative treatment, follow-up computed tomography revealed an expanded aneurysm of the MCA. We therefore performed coil embolization of the dissected MCA to prevent a rupture of the aneurysm. There has never been a reported case of acute dissection of the MCA immediately after EVAR that was not caused by catheters.

CONCLUSION: Hybrid OR with a fully equipped DSA system could provide precise and safe treatment strategies for cerebrovascular diseases.
