Comparison of Imaging and Clinically Relevant Features of Combined Hepatocellular Carcinoma and Cholangiocarcinoma with Hepatocellular Carcinoma. Med Sci Monit. 2019 Nov 14;25:8595-8601 Authors: Huang X, Li Y, Long L Abstract BACKGROUND The aim of this study was to compare the clinical, imaging, pathological, and prognostic characteristics of combined hepatocellular carcinoma and cholangiocarcinoma (cHCC-CC) and hepatocellular carcinoma (HCC). MATERIAL AND METHODS Medical records of 21 patients with cHCC-CC and 21 patients with HCC were retrospectively reviewed. Patients underwent a computed tomography (CT) examination within 1 month before surgery and did not receive preoperative interventional therapy. Clinical, imaging, pathological, and prognostic characteristics of cHCC-CC and HCC were compared. RESULTS On multi-phase contrast-enhanced CT, cHCC-CC could be differentiated from HCC based on the presence of a pseudocapsule (pA) and arterial phase and delayed phase (D>A) of CT enhanced scanning in HCC and cHCC-CC were statistically significant (both p50%), and a dilated bile duct, and when the CT value in the delayed phase is higher than in the arterial phase. VEGF expression is more likely to be positive in HCC than cHCC-CC. There was no significant difference between cHCC-CC and HCC in prognosis, but cHCC-CC was more likely to recur after treatment than HCC. PMID: 31726461 [PubMed - in process]

Conclusions: Our case demonstrates that histopathological defined diagnosis may significantly inform therapeutic decision-making in irresectable cholangiocarcinoma even in regard to conventional systemic therapy. A CT scan was performed on a 67-year-old man newly diagnosed with acute pancreatitis. The scan revealed a low-density lesion in the liver, a left renal nodule, and a right renal cystic mass. Intense 18F-FDG uptake was observed in the liver lesion and left renal nodule. No abnormal uptake was observed in the right renal mass. In addition, another focal intense uptake was observed in segment VII of the liver. Biopsies revealed intrahepatic cholangiocarcinomas in the 2 liver lesions, papillary renal cell carcinoma in the left renal lesion and clear cell renal cell carcinoma in the right renal lesion.

We report the case of a 55-year-old cirrhotic male patient admitted to our university hospital for dysphagia, revealing a 10 cm lower-third esophageal metastasis of an unresectable cHCC-ICC with stem-cell features. Computed tomography and abdominal magnetic resonance imaging scans revealed multiple hepatic lesions combining features of both HCC and ICC, associated with synchronous bone metastasis.
