Outcomes of allogeneic stem cell transplantation for DLBCL: a multi-center study from the Kyoto Stem Cell Transplantation Group

AbstractAllogeneic hematopoietic stem cell transplantation (allo-SCT) has been considered as a potentially curative treatment option for refractory or relapsed diffuse large B cell lymphoma (DLBCL) patients. However, there is little information available, especially for Japanese patients and in cord blood transplantation (CBT). We aimed to determine treatment outcomes of allo-SCT for DLBCL in the Kyoto Stem Cell Transplantation Group, a multi-institutional joint research group. Sixty-eight DLBCL patients who underwent their first allo-SCT between 2003 and 2016 were included. The median time from diagnosis to transplantation was 13.5 months. Thirty-one patients were in CR/PR at transplantation. Twenty-seven patients underwent CBT. The median follow-up for survivors was 44.2 months. Four-year overall survival (OS) and relapse-free survival (RFS) rates were 23% (95% CI, 13 –35%) and 20% (95% CI, 11–31%), respectively. Allogeneic hematopoietic stem cell transplantation (allo-HCT) is the only treatment option achieving long-term remission for patients with aggressive adult T cell leukemia –lymphoma (ATL)1,2. A recent meta-analysis showed that the pooled overall survival (OS) and relapse and non-relapse mortality rates were 40% [95% confidence interval (CI): 33‒46%), 36% (95%CI: 28‒43%), and 29% (95%CI: 21‒37%), respectively3. Previous studies reported that better pretranspla nt disease status4-7, fewer comorbidities8, and earlier transplantation9 are the most important factors in predicting allo-HCT success.

ConclusionPlerixafor proved effective to mobilize adequate numbers of PBSCs in individual patients with relapsed malignancies after prior single or tandem HDC+PBSCT. These PBSCs could establish sustained multi-lineage hematopoietic engraftment without any sequelae.

MicroAbstract: Plerixafor, a stem cell mobilizer with proven potential to rescue "poorly" mobilizing patients, was applied after salvage chemotherapy (+G-CSF) in a limited series of patients with potentially curable malignancies, who had relapsed after a previous autograft. This led to successful stem cell mobilization in all patients, who experienced sustained engraftment after further high-dose chemotherapy supported by the plerixafor-mobilized stem cells.

CONCLUSIONS: Median OS is somewhat prolonged in the last years with new treatment approaches but MCL is still an incurable disease. The first choice of treatment in MCL patients was R-CHOP. ConclusionChemotherapy followed by ASCT using a conditioning regimen of reduced melphalan and pharmacokinetically targeted busulfan is a promising strategy for treating patients with relapsed or refractory DLBCL who also have end-stage renal disease and are receiving hemodialysis.

In this study, we assessed the safety and efficacy of tandem autologous hematopoietic stem cell transplantation (auto-HSCT) strategy for adult T-LBL and evaluated prognostic factors affecting survival. 181 Newly-diagnosed adult T-LBL patients were enrolled, 89 patients were treated with chemotherapy alone, 46 patients were allocated to single auto-HSCT group, 46 patients were treated with tandem auto-HSCT.
