ConclusionThe PopPK/PD models showed numerically greater reductions in DAS28-CRP and ANC with sarilumab 200  mg every 2 weeks than with 150 mg every 2 weeks. There was no clinically meaningful influence of investigated covariates. These data contribute to the totality of evidence that supports a sarilumab subcutaneous starting dose of 200 mg every 2 weeks, with a subsequent reduction to 150 mg ever y 2 weeks in the event of laboratory abnormalities such as neutropenia.

A recent study compared three clinical criteria for rheumatoid arthritis (RA) in patients using certolizumab, assessing the predictability of treatment non-response. Researchers found Clinical Disease Activity Index measurement at three months may predict patient outcomes at 12 months...

Authors: Ceribelli A, Selmi C Abstract Genomic predisposition fails to fully explain the onset of complex diseases, which is well illustrated by the largely incomplete concordance among monozygotic twins. Epigenetic mechanisms, including DNA methylation, chromatin remodeling, and non-coding RNA, are the link between environmental stimuli and disease onset on a permissive genetic background in autoimmune and chronic inflammatory diseases. Autoimmune diseases now include almost 100 conditions and are estimated to cumulatively affect up to 5% of the world population with a healthcare expenditure superior to cancer wor...

ConclusionDuloxetine treatment in patients with RA in clinical remission appears to have comparable effectiveness on relieving remnant pain, improving activities of daily living and maintaining QOL. Duloxetine is not expected to have any effects on disease activity control.

AbstractThis letter describes the potential effect of B cell depletion on immune related adverse events associated with immune checkpoint inhibition. B cell depleting agents such as rituximab reduce B cell to plasma cell differentiation and antibody production. This treatment strategy is used in several immune mediated inflammatory diseases such as rheumatoid arthritis and small vessel vasculitis. The immune related adverse events associated with immune checkpoint inhibition resemble immune mediated inflammatory diseases. Here, we report a lower incidence of hypothyroidism in a trial of combined B cell depletion and immune...

CONCLUSIONS: While animal models have identified possible endocannabinoid pathways in arthritis, there is no clear evidence of benefit in humans or comparative efficacy with current treatments. At this stage, there is little evidence to support GPs prescribing cannabis-based medicinal products for arthritis. PMID: 32438375 [PubMed - in process]

Authors: Pablo AH, Breaud AR, Clarke W Abstract Immunosuppressant medications help suppress the immune system response through inhibition of various checkpoints in the regulatory biochemical pathway. This is useful in prevention of organ rejection in transplantation or in the treatment of autoimmune diseases such as lupus or rheumatoid arthritis. Quantification of immunosuppressive drugs in blood is needed clinically for optimization of treatment and to avoid toxicity or unwanted side effects. Here, we describe a quantitative method to determine the concentration of cyclosprine A, tacrolimus, sirolimus, and everoli...

In the largest observational study thus far investigating the drug hydroxychloroquine as a treatment for COVID-19, researchers found little evidence that it helps, and worrying evidence that the medication may cause harm. In a study published May 22 in the journal Lancet, scientists in the U.S. and Switzerland report on an analysis of more than 96,000 people hospitalized with confirmed COVID-19 in 671 hospitals on six continents. Nearly 15,000 patients were treated with one of the following: chloroquine (which is an older version of hydroxychloroquine), hydroxychloroquine, or either of those drugs in combination with an an...
