We present 4 cases of pathology-confirmed BIA-ALCL to illustrate varying presentations and imaging features of this disease process. Breast implant–associated anaplastic large cell lymphoma presents most commonly approximately 10 years after implantation of textured silicone or saline breast implants. Patients may present with breast enlargement, peri-implant effusion, a palpable breast or axillary mass, lymphadenopathy, breast skin erythema, or pain. Diagnosis of BIA-ALCL may be confirmed by sampling of peri-implant fluid or biopsy of peri-implant masses or regional lymph nodes.

Conclusions: Taking a lesson from composite breast augmentation, composite buttock augmentation is a new method for gluteal reshaping that seeks to use the best of both worlds: the reliable core projection of an implant combined with the natural appearance and feel of fat. Progressive instrument dissection seeks to minimize unnecessary dissection and to accurately and safely define the intramuscular pocket. The intramuscular placement of implants instead of fat should eliminate the risk of fatal fat pulmonary embolism in these patients. CLINICAL QUESTION/LEVEL OF EVIDENCE: Therapeutic, IV.

We report a case of breast implant-associated anaplastic large cell lymphoma in a 70-year old woman, who 21 years previously had undergone bilateral breast augmentation with textured silicone implants. Eleven years later, she had bilateral implant removal with possible implant leak and presence of ‘atypical cells’ in the seroma.
