Conformal Image Guided Radiotherapy (IGRT) including Stereotactic Body Radiotherapy (SBRT) is often used in the treatment of esophageal and pancreatic cancer and requires placement of fiducials for optimal tumor targeting with image guidance. SBRT delivers more efficacious localized RT with shorter treatment duration and fewer side effects than conventional RT. The standard of care for esophageal and pancreatic cancer involves initial endoscopic ultrasound (EUS) for tumor diagnosis and staging, followed by multidisciplinary clinic (MDC) evaluation to discuss need of chemotherapy with or without RT based on tumor staging.

Most hilar cholangiocarcinoma patients are not surgical candidates, leaving limited options of biliary stenting and chemotherapy. Adding photodynamic therapy (PDT) has been shown in prior studies to improve survival. Survival in one meta-analysis by Moole et al1 in cholangiocarcinoma patients showed median survival benefit of 7.6 months with addition of PDT. However, these studies focused on addition of PDT to stenting and chemotherapy mainly. Radiation therapy has been used in some cases of hilar tumors.

Conclusions: Hypothyroidism and difficult airways are a common sequel of RT. Selected cancer patients with subclinical hypothyroidism had a smooth perioperative course.

Palliation of dysphagia (Dys) in patients with inoperable esophageal cancer (EC) can be challenging. The major goal of palliation is to improve patient ’s quality of life (QoL) and Dys and allow adequate caloric intake. The most commonly used palliative modalities for Dys are radiation therapy (RT) and esophageal stenting. However, RT is limited by total dose and adverse events (AE) in patients receiving systemic therapy (ST), and stenting suffer s from a high rate of AE including reflux and chest pain.

Conclusion: Conversion surgery offers benefits in terms of increase survival for initially UR-PDAC for patients who responded favorably to chemo(radiation)therapy when combined with postoperative adjuvant chemotherapy.
