Case Example: Daisy Villa is a 3-star-rated skilled nursing facility (SNF) that is at the verge of losing its place in a local Accountable Care Organization (ACO) network as a result of high hospitalization rates of its short-stay patients. Through their quality assurance processes, the facility leadership conclude that lack of timely admission visits and medication reconciliation by physicians is a key contributory factor. When they approach the medical director to get help on the matter, she proclaims that poor outcomes are a result of “undue focus on profits rather than quality.” Disagreeing with the claim and disappointed, the facility leadership contemplates if the current medical director is the right person for the role.
