Sarah Murnaghan, the Philadelphia girl who underwent a lung transplant this month after a court battle, struggled after a breathing tube was removed Wednesday, so doctors "sedated and re-intubated" the girl, her mother said.

The implementation of effective prevention strategies has significantly reduced the incidence of cytomegalovirus (CMV) pneumonia after allogeneic hematopoietic stem transplantation (allo-HSCT)1; nevertheless, this clinical entity persists as a major clinical problem owing to poor survival, despite timely initiation of targeted antiviral therapy.1,2 Currently, diagnosis of CMV pneumonia still remains a challenge. Proven CMV pneumonia can only be diagnosed using virological, histopathological or immunochemistry methods on biopsied lung tissue1; this specimen type, however, is rarely obtained due to potential life-threatening complications.

Cystic fibrosis liver disease (CFLD) remains the third leading cause of death in patients with cystic fibrosis. Although most patients with CFLD present in childhood, recent studies suggest a second wave of liver disease in adulthood. There are no clear guidelines for diagnosing CFLD. Treatment options for CFLD remain limited, and while UDCA is widely used, its long-term benefit is unclear. Those who develop hepatic decompensation or uncontrolled variceal bleeding may benefit from liver transplant, either alone, or in combination with lung transplant.

CONCLUSION: Our results suggest that ESS is associated with a reduction in the frequency of antibiotic utilization for respiratory tract infections in lung transplant recipients with CF. Primary graft dysfunction (PGD) is the most important contributor to early morbidity and mortality in lung transplant recipients and reflects a combination of allograft ischemia-reperfusion injury, vasogenic pulmonary edema and inflammatory changes mediated by the innate immune system.1,2,3 Patients who develop PGD are at increased risk of poor lung function and death.4-6 There are limited therapeutic options available to patients who develop PGD after lung transplantation and as such, the main focus has been on the identification of risk factors for PGD and prevention strategies.
