Is the Stethoscope Dying? High-Tech Rivals Pose a Threat to the Symbol of the Medical Profession

CHICAGO (AP) — Two centuries after its invention, the stethoscope — the very symbol of the medical profession — is facing an uncertain prognosis. It is threatened by hand-held devices that are also pressed against the chest but rely on ultrasound technology, artificial intelligence and smartphone apps instead of doctors’ ears to help detect leaks, murmurs, abnormal rhythms and other problems in the heart, lungs and elsewhere. Some of these instruments can yield images of the beating heart or create electrocardiogram graphs. Dr. Eric Topol, a world-renowned cardiologist, considers the stethoscope obsolete, nothing more than a pair of “rubber tubes.” It “was OK for 200 years,” Topol said. But “we need to go beyond that. We can do better.” In a longstanding tradition, nearly every U.S. medical school presents incoming students with a white coat and stethoscope to launch their careers. It’s more than symbolic — stethoscope skills are still taught, and proficiency is required for doctors to get their licenses. Over the last decade, though, the tech industry has downsized ultrasound scanners into devices resembling TV remotes. Syncope accounts for a large proportion of Emergency Department visits each year, with an overall incidence of 0.6% per year in adults and 2-6% in elderly patients. Cardiac syncope, which accounts for 5-21% of syncope presentations in primary care and emergency settings, is transient loss of consciousness due to a reduction in cardiac output secondary to cardiopulmonary disease (i.e., mechanical/structural heart disease or arrhythmias). Distinguishing cardiac syncope from other causes, such as reflex syncope or orthostatic syncope, can be challenging.

The demonstrated effectiveness of implantable cardioverter-defibrillators (ICDs) for primary and secondary preventions of sudden cardiac death has resulted in an increasing number of patients presenting with recurrent, appropriate ICD shocks for ventricular tachycardia VT.(1) In a selected group of VT patients such as those included in the study by Bradfield and colleagues (2) in this issue of Heart Rhythm, autonomic modulation is considered as adjunctive treatment option after failed catheter-based VT ablation and anti-arrhythmic drug therapy.
