To evaluate the results of the concurrent use of lenalidomide-dexamethasone with intensity-modulated radiation therapy (IMRT) for solitary plasmacytoma in terms of toxicity and outcomeMaterials&Methods: Forty-six patients were treated for histologically proven SP between June 2007 and June 2018 in our Department (XXX). All patients received IMRT. The median total dose was 40 Gy (range, 40-46). Prescription of concurrent lenalidomide-dexamethasone with radiotherapy was left discretion of the referring hematologist-oncologist and started the first day of radiotherapy for four cycles.

CONCLUSION: Radiation therapy is the standard treatment for solitary bone plasmocytoma. It ensures good local control in 90% of cases. The prognosis is affected by progression to multiple myeloma, which justifies rigorous monitoring after treatment and suggests a reflection on the exact place of chemotherapy. We present a 63-year-old Japanese female with PCM positive for t(14;20). The tumor responded well to a proteasome inhibitor, bortezomib, and the patient achieved complete remission. Six months after remission, tumor relapse was noted in the left cerebellum and the right frontal lobe of the cerebrum. After whole brain radiation therapy, the tumor masses decreased in size. We report a 72-year-old male with testicular infiltration as extramedullary disease of IgD λ-type multiple myeloma. The patient received systemic treatment, which included high orchiectomy, anticancer chemotherapy, and radiation therapy for bone metastasis. Conclusion: This retrospective series of patients treated with palliative intent for multiple myeloma using various dose and fractionation schemes showed favorable symptomatic relieve in most patients. Radiographic response did not correlate with clinical response with fewer patients having radiologic disease regression. Longer follow-up is necessary to determine if the lack of radiologic response is associated with clinically relevant recurrent pain.
