Impact of hypogonadotropic hypogonadism on ovarian reserve and response

AbstractObjectiveTo evaluate the hormonal profile, antral follicle count (AFC) and ovarian response of patients with hypogonadotropic hypogonadism (HH).DesignObservational retrospective cohort including infertile women with HH undergoing assisted reproductive treatment (ART).SettingUniversity-affiliated infertility center.Patient(s)Thirty-three women with HH who underwent ART between January 2007 and September 2018. The control group comprised 66 age-matched counterparts with tubal or male factor infertility. The patients with an abnormal karyotype, and those presenting primary or secondary amenorrhea due to other causes, were cautiously excluded.Intervention(s)None.Main outcome measure(s)The primary outcome was serum levels of anti-M üllerian hormone (AMH) and AFC. We also investigated whether HH impacts ovarian response and reproductive outcomes.Result(s)Although AFC was similar between groups, HH patients showed significantly higher AMH levels (4.6  ± 2.7 ng/mL vs. 3.0 ± 1.9,p = 0.010) and lower basal FSH and LH. ConclusionWithin the limitations of the study, GBR with or without autogenous block graft may be an effective approach in augmenting horizontally deficient mandibular or maxillary ridges, before the placement of dental implants. However, more complications may be seen with the use of an autogenous block graft related to the donor sites.

Conclusion: Chinese women had a substantially lower risk of venous thromboembolism during pregnancy and the postpartum period compared to that of Caucasians. The occurrence of pregnancy-related venous thromboembolism was largely confined to the early pregnancy period, probably related to the adoption of thromboprophylaxis, a lower rate of Cesarean section, and early mobilization. CONCLUSIONS: Because of its pleiotropic roles in several reproductive events and its association with some life-threatening pathological conditions of pregnancy, bikunin may represent a non-invasive marker for improving follow-up and early diagnosis. Studies showing its usefulness as a drug for reducing the risk of preterm delivery and improving neonatal outcomes have yielded interesting results that deserve to be investigated through further research.
