Acute commissural rupture in a giant aortic root aneurysm

Here, we reported one case of commissural rupture in the presence of a giant root aneurysm and a history of chronic aortic regurgitation, which emphasized the diagnostic value of high-quality 3-dimensional reconstructions of multimodality imaging in assessing the cause of abruptly deteriorating valve regurgitation.

ConclusionsIn experienced centers, Ross procedure is a reliable alternative to prosthetic or homograft valve replacement in young adults suffering from active aortic valve endocarditis, with a low operative risk and good long-term results.

We describe a successful bioprosthetic annular stretching in a patient with severe prosthetic aortic valve stenosis from a degenerated 19-mm Mitroflow valve (Sorin Group USA Inc, Arvada, CO, USA). This technique allowed for implantation of a 23-mm Evolut-R Pro valve (Medtronic, Minneapolis, MN, USA) with significant improvement in hemodynamics after prosthetic annular stretching. We have also summarized other case series and case reports which have previously described similar techniques.
