Premorbid Hemostasis in Women with a History of Pregnancy Loss.

CONCLUSION:  These results indicate that chronic hypercoagulability and impaired clot contraction constitute a premorbid status in patients with pregnancy loss. The data confirm a significant pathogenic role of hemostatic disorders in pregnancy loss and suggest the predictive value of thrombodynamics and blood clot contraction assays in evaluating the risk of pregnancy loss. CONCLUSION: Thrombophilias are inherited or acquired predispositions to vascular thrombosis and have been associated with thrombosis of the arteriovenous fistula. Patients on hemodialysis with recurrent vascular access thrombosis and presence of thrombophilia should be evaluated about the need for anticoagulant therapy with a vitamin K antagonist. Women with inherited bleeding disorders (IBDs) experience significant challenges in their health, personal and reproductive lives. Heavy menstrual bleeding (HMB), general bleeding symptoms, iron deficiency and excessive bleeding during miscarriage, pregnancy and delivery are all common complications for women with IBDs. Symptoms may present in childhood or adolescence, often with significant delays to definitive diagnosis and appropriate treatment due to lack of recognition of the prevalence and impact of IBDs in women.

The role of genetic thrombophilia screening for identifying a hypercoagulable state in the management of venous thromboembolism. We searched MEDLINE and EMBASE from 1995 to 2017, the websites of the professional bodies including American Society of Hematology, British Society of Hematology, International Society of Thrombosis and Hemostasis, College of American Pathologists, American College of Medical Genetics, and American Society of obstetrics and gynecology for their clinical practice guidelines. We used search strategy terms – venous thromboembolism, inherited, thrombophilia, and hypercoagulable state. Thromboph...

Rationale: Essential thrombocytosis (ET) is a myeloproliferative neoplasm characterized by clonal proliferation of the megakaryocytic lineage within the bone marrow and phenotypically by an elevated platelet count in peripheral blood. Common vascular complications include thrombosis, microvascular disturbances, and hemorrhage. ET with recurrent spontaneous abortion as the primary symptom is rare. Patient concerns: A 30-year-old pregnant woman (gestational age: 8 weeks) with a history of recurrent spontaneous abortion in the mid trimester was admitted to our hospital for further management. Diagnosis: The diagnosis ...

Conclusions: Association between 2 procoagulant conditions, inherited thrombophilia and pregnancy, has an important impact for the mother and fetus. This review will summarize the impact of each inherited prothrombotic factor on cardiovascular and pregnancy outcomes and will discuss the role of anticoagulation treatment for women diagnosed with inherited thrombophilia.

AbstractWhatever the outcome, pregnancy provides the opportunity to offer effective contraception to couples motivated to avoid another pregnancy. This narrative review summarises the evidence for health providers, drawing attention to current guidelines, on which contraceptive methods can be used, and when they should be started after pregnancy, whatever its outcome. Fertility returns within one month of the end of pregnancy unless breastfeeding occurs. Breastfeeding, which itself suppresses fertility after childbirth, influences both when contraception should start and what methods can be used. Without breastfeeding, eff...

Conclusion Figure 4 shows that the 27 different molecular defects within the ALB so far reported to cause CAA are located in ten different exons (1, 3, 4, 5, 7, 8, 9, 10, 11, and 12) and in seven different introns (1, 2, 3, 6, 10, 11, and 12) (The Albumin Website, 2018; Caridi et al., 2019). Variations in the last two coding exons (13 and 14) would probably cause the presence of a circulating C-terminal variant of the protein and not CAA. The first twelve exons of ALB, with the exception of the two shortest, exons 2 and 6, were reported to contain at least one molecular defect resulting in CAA (The Albumin Website, 2018; ...
