Shunting between the left ventricle and right atrium can be produced by straddling tricuspid valve with dual orifices

In the current issue of the journal, Guenther and colleagues describe their experience with a rare combination of findings, which produced a shunt from the left ventricle to the right atrium, along with so-called “double outlet right atrium”.1 There are two ways of dealing with such an unexpected constellation of findings. One way is to use the principle known as Occam's razor, and opt for the simplest explanation. The other is to invoke as yet unproven developmental hypotheses, and still further complic ate the situation.
