Day surgery for children

This article summarizes the essential components needed to provide a high-quality paediatric day surgery service. It also highlights the importance of good preoperative assessment, which involves having rigorous inclusion and exclusion criteria, but also comprising some degree of flexibility allowing evaluation of children on an individual basis. Strategies to minimize postoperative pain and nausea and vomiting are also discussed, with emphasis being placed on the safe discharge of these children home.

ConclusionResults from our study supported the idea that the release of mPFC and NAc exosomes of CCI model has elevated the pain sensations in the subjected mice. This study will further help in designing new clinical trials, and will revolutionize the drug-induced anesthetic responses.

A 44-year-old woman presented with lumbar pain (visual analog scale [VAS]  = 9) secondary to adrenocortical carcinoma sclerotic L4-vertebral metastasis. The positron emission tomography-computed tomography hypermetabolic lesion (Fig 1a,d) was unresponsive to radiotherapy or chemotherapy. She refused surgery. Following general anesthesia and aseptic preparation, 2 power d rill-mounted (Aesculap, Center Valley, Pennsylvania) k-wires were inserted in each vertebral pedicle with lateromedial opposite directions, resulting in crossed wires.

Insufficient perioperative pain treatment is known as a highly predictive risk factor for the development of chronic postoperative pain. Remifentanil is an ultrashort-acting opioid that provides quick and efficient analgesia but is associated with the induction of opioid-induced hyperalgesia. Despite these well-known characteristics, this substance is being increasingly used in anesthesia and in a variety of medical fields, such as intensive-care medicine and obstetrics. The aim of our study was to reveal whether remifentanil influences postoperative pain, the requirement for postoperative analgesics, and requirement of an...

AbstractBackgroundExcessive opioid prescriptions after orthopedic surgery are common. The purpose of this study was to examine the association between surgical severity, preoperative opioid use and patient characteristics with postoperative opioid prescriptions and refills.MethodsSeventy-nine patients undergoing orthopedic surgeries were reviewed. Surgical pain severity was categorized as mild (n  =  25), moderate (n  =  26) and severe (n  =  28). Patients were also categorized as opioid naïve (n  =  57), experienced (n  =  16) and tolerant (n &thin...

This study aimed to assess the perioperative outcomes of adjunctive hypnotherapy undergoing transfemoral TAVI with LACS.Consecutive patients (n = 143) with symptomatic severe aortic stenosis who underwent transfemoral TAVI with LACS only (n = 107) or with LACS and hypnotherapy (n = 36) between January 2015 and April 2016 were retrospectively included in the study. The clinical outcomes were compared between the two groups. The LACS with hypnotherapy group had a significantly shorter length of stay in the intensive care unit (ICU; LACS only versus LACS with hypnotherapy: 4.0 (4.0-5.5) days versus 3.0 (3.0-5.0) days, P

Periarticular injection or anesthesiologist-performed adductor canal block are commonly used for pain management after total knee arthroplasty (TKA). A surgeon-performed, intra-articular saphenous nerve block has been recently described. There is insufficient data comparing the efficacy and safety of these methods.
