63-Year-Old Woman With Abdominal Pain and Thrombocytopenia

A 63-year-old-woman presented to the emergency department with acute-onset, 8 out of 10, stabbing lower-abdominal pain associated with nausea and vomiting. The pain was constant and present in both lower quadrants. She denied diarrhea, constipation, hematemesis, and hematochezia/melena. She had presented to her family physician 5 days earlier with dysuria. At that time, urine dipstick was positive for nitrites and culture grew Escherichia coli (E.  coli), and she was prescribed a 7-day course of nitrofurantoin for a urinary tract infection (UTI).
