Prediction of complication related death after radical cystectomy for bladder cancer with machine learning methodology.

Conclusion: This study with a novel methodological approach adds CHF and chronic pulmonary disease to previously known independent prognostic risk factors for early death after RC. Importantly, the risk prediction tool uses purely pre-operative data and can be used before surgery. PMID: 31552774 [PubMed - as supplied by publisher]

Discussion: The Z-track method minimizes fluid leakage from the puncture site. Injecting medication into the skin using this method is important to preventing post-procedure leaking. Once a needle has entered subcutaneous tissue and muscle, it opens a track that may not reseal immediately. There are also studies suggesting that Z tracks may reduce pain during injection. We suggest using the method during your paracentesis procedure.Z tracks are used for all kinds of intramuscular injections and can be applied to other sites on the body. Pull and press the skin and tissue 2 cm caudad to the deep abdominal wall and insert th...

AbstractPurpose of ReviewThiazolidinediones (TZDs) are the only pharmacologic agents that specifically treat insulin resistance. The beneficial effects of TZDs on the cardiovascular risk factors associated with insulin resistance have been well documented. TZD use has been limited because of concern about safety issues and side effects.Recent FindingsRecent studies indicate that cardiovascular toxicity with rosiglitazone and increase in bladder cancer with pioglitazone are no longer significant issues. There are new data which show that pioglitazone treatment reduces myocardial infarctions and ischemic strokes. New data co...

Rayfield Byrd knows when it’s time to wake up every morning. The 68-year-old Oakland, Cal., resident hears a voice from the living room offering a cheery good morning. Except Byrd lives alone. A little after 8 a.m. each day, a small yellow robot named Mabu asks Byrd how he’s doing. Byrd has Type 2 diabetes and congestive heart failure, and about three years ago, he had surgery to implant a microvalve in his heart to keep his blood flowing properly. To stay healthy, he takes four medications a day and needs to exercise regularly. To make sure his heart is still pumping effectively, his doctor needs to stay on to...

(University of Alabama at Birmingham) Doxorubicin is a chemotherapy drug used in ovarian, bladder, lung, thyroid and stomach cancers, but it carries a harmful side effect. The drug causes a dose-dependent heart toxicity that can lead to congestive heart failure. Researchers have found an important contributor to that heart pathology -- disruption of the metabolism that controls immune responses in the spleen and heart. This dysregulated immunometabolism impairs resolution of inflammation, and chronic, non-resolving inflammation leads to advanced heart failure.

Abstract PURPOSE: To determine whether the rate of venous thromboembolism (VTE) following radical cystectomy (RC) is changing overtime. MATERIALS AND METHODS: The American College of Surgeons National Surgical Quality Improvement Program database was used to identify patients who underwent RC for bladder cancer from 2011 to 2016. VTE was defined as pulmonary embolism or deep vein thrombosis within 30 days of RC. VTE rate by year was assessed using the Cochran-Armitage test for trend. Associations between patient features and VTE were evaluated with multivariable logistic regression. RESULTS: A total of 8...

CONCLUSION: This article systematically reviews PPARγ's mechanisms of action and multilayer regulations. In addition, novel classes of therapeutics modulating PPARγ and new direction of research on genetic variants that affect PPARγ function and antidiabetic drug response are highlighted, which sheds light on PPARγ as a promising target for developing safer and precision medicine based therapeutic strategies. Publication date: September 2016 Source:Pharmacological Research, Volume 111 Author(s): Shuibang Wang, Edward J. Dougherty, Robert L. Danner Peroxisome proliferator-activated receptor gamma (PPARγ) is a ligand-activated nuclear receptor that regulates glucose and lipid metabolism, endothelial function and inflammation. Rosiglitazone (RGZ) and other thiazolidinedione (TZD) synthetic ligands of PPARγ are insulin sensitizers that have been used for the treatment of type 2 diabetes. However, undesirable side effects including weight gain, fluid retention, bone loss, congestive heart failure, and a possible inc...
