Guidelines Are Perishable Goods that Can Go Bad Quickly

The article by Yamaguchi et  al.1 describes a national claim based registry study from Japan in which the outcomes of ruptured AAA (rAAA) were analysed, focusing on elderly (aged ≥ 80 years) patients. Almost 4000 patients were included, half aged> 80 years. Elderly patients with rupture were less likely to be treated, and those treated had a worse outcome compared with younger patients. However, for the subgroup of patients selected for EVAR, no significant difference was seen in short term outcome between elderly and younger patients.
