A 68-year old man with a history of hypertension, diabetes, coronary artery disease and an obliterating lower limbs arteritis, presented for a stroke. Electrocardiogram did not show atrial fibrillation nor ischemic electrocardiographic signs. White blood cell count (10800/m3) was high but eosinophil count (234/mm3) was normal. Postoperative atrial fibrillation (POAF) after cardiac surgery remains a highly prevalent and costly condition that negatively impacts patient quality of life and survival. Numerous retrospective studies, meta-analysis, and review papers have been reported identifying POAF risk based on patients ’ risk factors and clinical biomarkers. In this narrative review, the authors report significant variations among selected pre- and perioperative biomarkers used to predict POAF incidence in patients without a history of atrial fibrillation (AF).

ConclusionA considerable burden of previously unknown AF was detected when long-term monitoring was applied in at-risk patients. Biomarkers were associated with AF incidence and improved prediction of long AF episodes.
