Hemodynamic Effects of Late Sodium Current Inhibitors in a Swine Model of Heart Failure

Ranolazine, a potent and specific inhibitor of late sodium current (INa,late), is commonly used to treat angina pectoris or suppress arrhythmia.1-4 Mexiletine, also a potent inhibitor of INa,late, is approved to suppress arrhythmia.5,6 Some patients receiving these drugs are likely to have heart failure (HF) due to ischemia or other causes of myocardial mechanical dysfunction. The consequences of INa,late inhibition have not been studied in patients with advanced Mattiello JA, Margulies KB, Jeevanandam V, Houser SR.
