Neuroaspergillosis as the presenting sign of a chronic granulomatous disease

A 13-year-old girl presented with a two-month history of intermittent low-grade fever and tickling in her left hand and left side of the face. At the physical examination, left-sided paresthesia and hyposthenia in upper extremities were present. Routine laboratory tests and viral and bacterial assays were negative; antibody titers of Aspergillus species were positive. At brain magnetic resonance, numerous gadolinium-picking nodules were found bilaterally, with perilesional edema and moderate midline shift (Figure 1a,b).
