A vasculitic rash: Could it be scurvy?

A 33-year-old man with a history of Crohn ’s disease and previous bowel surgery was referred to the dermatology clinic with a 6-month history of fatigue and 3-month history of a petechial rash of the legs associated with right leg swelling and spontaneous bruising. Examination revealed perifollicular haemorrhage with hyperkeratosis and co rkscrew hairs to the legs bilaterally. There was tense swelling of the right leg with significant ecchymosis to the right posterior thigh. On further questioning, the patient admitted to eating tuna pasta exclusively and avoided fibre as to prevent exacerbation of his symptoms of Crohn’s disease.
