An adult with periorificial and acral erythema with erosions

Background: Acrodermatitis enteropathica (AE) may be inherited as a rare autosomal recessive defect in zinc absorption or be acquired due to insufficient dietary intake or malabsorption. Sometimes the triad of diarrhea, depression, and dermatitis aids in the diagnosis of this entity. Nevertheless, in an adult patient with multiple comorbidities and multifactorial etiologies for diarrhea and mood changes, a thorough history and skin exam is essential for diagnosis.

The physical examination revealed moderate tenderness of the epigastric region; the laboratory evaluation showed leukoc...

We report the case of a 48-year old woman with long-standing lepromatous leprosy of 15 years of evolution, with a bacterial index of 4 in the direct smear and the initial skin biopsy. The patient was treated with multidrug therapy for 32 months although the treatment recommended by the World Health Organization (WHO) is only for 12 months. A skin biopsy was taken to determine if there was an active disease. We observed a diffuse dermal inflammation with numerous foreign body giant cells and vacuolated macrophages (Virchow´s cells). These cells contained granular acid-fast material that was also positive with immunohi...

CONCLUSIONS: Although ART scale-up was temporally associated with a decline in TB notification rates, the achieved rates of decline are below those required to achieve the End TB Targets. Additional investments in tuberculosis control should include efforts to promote earlier care seeking and ART initiation among HIV-positive persons. PMID: 31529618 [PubMed - in process]
