Gastrointestinal Manifestations of Pediatric Autonomic Disorders

Functional gastrointestinal disorders (FGIDs) are currently classified under the Rome criteria based on symptoms and absence of organic disease. Preliminary studies have shown that FGIDs are probably not restricted to the GI tract, but may represent a systemic disorder with comorbidities affecting other parts of the body, including migraine, fatigue, aches and pains, etc. The autonomic nervous system (ANS) provides the extrinsic control of GI motility, secretions, and even immune response. The role of the ANS in the development of FGIDs and comorbidities is still unclear. Limited data demonstrate orthostatic intolerance such as reflex syncope and postural tachycardia syndrome in a large subset of subjects with FGIDs. Some studies have found improvement in the GI symptoms with treatment of the orthostatic intolerance it produces. Prospective studies are needed to determine the chronology of the development of the comorbidities, the triggers that induce these syndromes, and effective treatments. This chapter aims to review current understanding of the role of the ANS in FGIDs.

We describe 5 independent Italian subjects presenting with progressive limb girdle muscular weakness, brain white matter abnormalities, merosin deficiency and LAMA2 gene mutations. We detected 7 different mutations, 6 of which are new. All patients showed normal psicomotor development and slowly progressive weakness with onset spanning from childhood to forties. CONCLUSION: This case reports a rare combination of cluster headache and harlequin sign that warrants further testing to exclude pathology in the brain, neck or mediastinum. CONCLUSIONS: The present study does not support an acute effect of nVNS on descending pain inhibition, pain intensity perception or supraspinal nociception in healthy adults. Hypotension during spinal anaesthesia for caesarean delivery is a common problem, occurring in 70-80%1 of normotensive parturients. The clinical effects of severe intra-operative hypotension are not benign and range from maternal nausea, vomiting and headache to fetal acidosis, neurological injury and weak sucking reflexes.2,3 Spinal anaesthesia-induced hypotension results from blockade of thoracolumbar sympathetic outflow and decrease in systemic vascular resistance, with hypotension exacerbated by impaired venous return to the heart due to vena caval compression and venous pooling in the lower limbs.
