Improving Safety and Handoff for the Fast Tracked Patient
Following an endoscopy procedure, most patients are fast tracked from the endoscopy suite to phase II. Some patients were returning to phase II from the endoscopy suite that had not achieved phase II status by evidence of decrease level of consciousness, difficult arousing, and unstable vital signs.
CONCLUSIONEsophageal perforation is a very rare complication of TEE. High suspicion is mandatory to reach prompt diagnosis and install effective treatment. Primary closure of the perforation is the treatment of choice, and the endoscopic approach is a safe and feasible option in high volume centers.

MR imaging plays a crucial role in the post-CRT assessment of rectal cancer; results are used for treatment planning. Radiologists should assess response or progression, possibility of a complete response, risk factors for incomplete resection, and nodal stage. T2-weighted MR imaging with diffusion-weighted imaging yields the best results to identify a complete response, but endoscopy is also very important. Overstaging of transmural and MRF invasion after CRT occur regularly, owing to residual stranding regarded as tumor to err on the safe side. Nodal restaging is a challenge. A structured report format or checklist is recommended.
Endocuff improves detection at colonoscopy, but seems to impede terminal ileal (TI) intubation. We assessed the impact of Endocuff Vision (EV) on TI intubation using adult or pediatric colonoscopes.
