To date, only few reports have focused on mediastinal synovial sarcoma imaging findings. Herein, we report a case of a 13cm primary mediastinal giant synovial sarcoma, diagnosed in a 56-year-old patient admitted in our Department of Radiology with a six-month history of dyspnea and back pain.
