ConclusionThe laparoscopic reverse technique is an alternative approach to face retrocervical or rectovaginal nodules infiltrating the anterior rectal wall. In this technique, the separation of the nodule from the rectal wall is performed at the end of the surgery and not at the beginning as performed within the traditional technique. It is true that large analyses such as this should be examined critically and with an understanding of the limitations of large data sets. Regarding surgery duration, we did not adjust the analysis by surgery duration or use of regional blocks. While these could potentially be confounding factors related to pain in the immediate postoperative period and use of opioids, they would be unlikely to contribute to prolonged opioid use, which was the primary outcome analyzed in this study. CONCLUSIONS: We were able to define predictors of admission for patients having LH/RAH managed with an ERAS pathway. Increased ASA physical status, being African American, and increased length of procedure were significantly associated with admission after LH/RAH performed under an ERAS pathway. In addition, the incidences of urgent clinic and ER visits, readmissions, and reoperations within 90 days of surgery were similar for patients who were discharged on the day of surgery compared to those admitted. CONCLUSIONS: This study shows that a low dose combination of ketorolac and tramadol is sufficient to block the pain responses induced with a needle holder in pigs 20 minutes after its administration.
