The Invited Editorial by Naylor1 begins with a quote from Friedrich Nietzsche highlighting the difference between facts and interpretations, and that difference is the key to this discussion on duplex ultrasound surveillance and management of restenosis after carotid endarterectomy and stenting. Naylor is responding to an Editorial by Paraskevas and Gloviczki,2 who reviewed the current clinical evidence and recommended routine and relatively frequent duplex ultrasound surveillance after carotid interventions, as described in the recently published Society for Vascular Surgery (SVS) practice guidelines on follow-up after vascular surgery arterial procedures.
