Acute aortic dissection mimicking a gastric ulcer: Medico ‐legal implications

The diagnosis of aortic dissection is often difficult because the symptoms are usually non ‐specific. AD should be considered in the differential diagnosis of all cases of idiopathic retrosternal pain. Misdiagnosis of AD is a common cause of legal suits in medical practice. Prevention requires a complete and thorough evaluation. AbstractThe diagnosis of aortic dissection is often difficult because the symptoms are usually non ‐specific. Rationale: Oral ingestion of glyphosate can induce gastrointestinal symptoms such as vomiting, abdominal pain, and hematochezia. Timely and effective treatment of pyloric stenosis caused by glyphosate poisoning is important. Patient concerns: The patient had a poor appetite, accompanied by nausea and vomiting of a small amount of dark brown material that resembled blood clots several times a day. Gastroscopy revealed gastric ulcer, a large pyloric antrum ulcer, and a deformed stomach cavity. Diagnosis: Pyloric stenosis due to glyphosate poisoning in a 36-year-old man. Interventions: The patients received distal ...

Abstract Myristica fragrans seeds are traditionally used to treat dyspepsia, vomiting and abdominal pain. The purpose of this study was to investigate the protective effect of Myristica fragrans in ethanol induced gastric ulcer. Study was carried out on rats, divided into four groups; negative control, positive control, standard drug control, and Myristica fragrans extract treated rats. The pH, ulcer index, acidity values and histopathological examination of stomach were evaluated. Myristica fragrans significantly (P

A 59-year-old man with type 2 diabetes presented at our hospital because of epigastric pain and nausea. He had a history of gastric ulcers and had undergone Helicobacter pylori eradication therapy 2 years before presenting at  our hospital. EGD showed only ulcer scars in the stomach (A). Esomeprazole (20 mg/day), the proton pump inhibitor taken by the patient for GERD, was changed to a potassium-competitive acid blocker (vonoprazan, 20 mg/day).

Paracetamol (acetaminophen, APAP) is one of the most frequently used analgesic agents worldwide. It is generally preferred over nonsteroidal anti-inflammatory drugs because it does not cause typical adverse effects resulting from the inhibition of cyclooxygenases, such as gastric ulcers. Nevertheless, inhibitory impact on these enzymes is claimed to contribute to paracetamols mechanisms of action which, therefore, remained controversial. Recently, the APAP metabolites N-arachidonoylaminophenol (AM404) and N-acetyl-p-benzoquinone imine (NAPQI) have been detected in the central nervous system after systemic APAP administrati...

A 75-year-old Ethiopian-born woman (emigrated aged 48 years) was admitted with acute recurrent vomiting and mild abdominal pain. She had long-standing orally treated diabetes, hypertension and glaucoma. Examination, ECG, chest and abdominal X-rays were unremarkable save for tachycardia 130/min. Hb was 10.2 g/dl, blood urea nitrogen (BUN) 18 mg/dl, creatinine 1.6 mg/dl (previously 1.0), C-reactive protein (CRP) 30.4 mg/dl, normal amylase and liver enzymes. Her ‘gastroenteritis’ improved with fluids and she was quickly discharged, only to be readmitted within 6 days with anorexia and diffuse abdominal pain. Exami...

Abstract Sacran, a new polysaccharide isolated from cyanobacterium Aphanothece sacrum (Sur.) Okada, is known to have potential as an active pharmaceutical ingredient (API) for the treatment of atopic dermatitis, various types of dermatitis, skin wound, hemorrhoids and corneal epithelium disorder. In the present studies, the effects of sacran on skin injury and skin pain induced by stratum corneum tape-stripping and gastric ulcer induced by indomethacin, a non-steroidal anti-inflammatory drug (NSAID), and hydrochloride/ethanol (HCl/EtOH) in mice were investigated. Sacran solution 0.05% (w/v) showed greater reductio...
