Spanish Registry of Recent-onset Psoriatic Arthritis (REAPSER study): Aims and methodology

ConclusionsThe REAPSER study includes a cohort of patients with recent-onset PsA, before the disease course was modified by disease-modifying antirheumatic drugs prescribed in rheumatology departments. Exhaustive information collected in each visit is expected to be an important data source for future analysis.

AbstractStreptococcus is well associated with a myriad of inflammatory diseases. Among others, this bacterium is linked to the triggering of psoriasis and to post-streptococcal reactive arthritis (PSRA), an arthritis which is typically confined to peripheral joints. Three patients who developed acute psoriatic spondyloarthritis (SpA) following a recent streptococcal infection are described in this article. We searched the existing literature for cases of axial involvement in PSRA and reviewed the association between streptococcal infection and psoriasis or psoriatic arthritis )PsA). In all patients, psoriatic SpA occurred ...

Conclusions: PET has the advantage of being able to detect degenerative joint changes at a molecular level even before the structural changes have taken place. Hence, it has superiority over other conventional imaging modalities to detect arthritis in the early stages as well as investigate multiple joints in a single scan. As the resolution of PET is relatively low, combining PET with CT provides structural information to better assess the joint. Quantifying disease activity has the benefit of surveilling disease activity over the course of time and monitoring treatment response, remission and relapse of the disease.

Publication date: Available online 15 May 2019Source: Reumatología Clínica (English Edition)Author(s): Mayra Rauseo Vera, Luis Arturo Gutiérrez-González, Irama Maldonado, Soham Al SnihAbstractIntroductionSpondyloarthropathies (SpA) are disabling diseases with a prevalence of 1.9% in the general population. The indices designed for monitoring the disease should be valid, reliable and cross-culturally adapted for decision-making concerning the appropriate treatment. Changing an adjective or pronoun in a self-administered questionnaire could be the big difference in condensing an idea in a few word...

Conclusions Sacroiliitis can be seen in patients with FMF during childhood, and M694V mutation seems to be a susceptibility factor for its development. Inflammatory low-back pain and leg and heel pain could suggest sacroiliitis.

A previously healthy, well-appearing 42-year-old female living in a modern, high-rise apartment in downtown Los Angeles calls 9-1-1 at 5:30 am complaining of worsening of a burning, epigastric pain she had been experiencing for the last three days. She reports associated nausea and non-bloody, non-bilious vomiting, and that she couldn’t manage to get comfortable in bed until she finally decided to call for help at daybreak. During her 9-1-1 call, she reports “pain, like heartburn, that just woke me up again and I had to throw up, … and then I was sweating so much.” Using the Los Angeles Tiered Disp...

Danish philosopher and theologian Søren Kierkegaard once wrote, “Life can only be understood backwards; but it must be lived forwards.” The beginning of a new year is an appropriate time to look back over the lessons learned from the mistakes and experiences of previous months. In 2018, I wrote two letters of resignation, grieved the ending of two significant relationships, and spent several weeks on a hospital waiting list for severe depression and weight loss. I fumbled, got lost, and confronted demons that I had been running from my entire life. The result is that I came away with a set of invaluable ...

ConclusionsSpinal diffuse idiopathic skeletal hyperostosis is a common bone-forming disorder in the middle-aged and older population. Its recognition is important because this condition can render the spine susceptible to fractures even with minor trauma, such as ground-level fall, resulting in delayed diagnosis and neurologic deterioration.

ConclusionsThe thickening of the pulleys in the flexor tendons is an easy-to-detect sign with good sensitivity for the diagnosis of PsA. Its specificity and positive predictive value are not very high; however, if it is included in a complete classification process, sonographers should report it during hand evaluations of patients with arthritis.
