A Central Role of Radiation Therapy in Central Nervous System Germinoma

Central nervous system germinomas are generally chemo- and radiosensitive, and these modalities are the mainstays of care. Although emergent operative decompression was warranted given the patient's presentation with obstructive hydrocephalus, upfront intra-arterial chemotherapy was nonstandard. Our team would have recommended 4 cycles of carboplatin and etoposide followed by restaging and response-based whole ventricular radiation therapy with an involved field boost, as per the closed-to-accrual Children's Oncology Group protocol ANCS1123, stratum 2.
