Type 2 diabetes (T2D) is associated with higher bone mineral density (BMD) [1], which is supposed to be associated with lower risk of fracture in the general population. However, individuals with T2D actually have higher risk of fracture, particularly hip fracture [2] and vertebral fracture [3]. Our previous study showed that approximately half of all fractures occurred in those do not have low BMD [4]. This suggested that fractures in patients with diabetes may result from alterations in skeletal properties or bone quality not captured by dual-enery X-ray absorptiometry (DXA)- derived BMD [5].

Purpose of review Adrenal tumors occur in 5% of population with higher prevalence in elderly. Patients with adrenal tumors present with overt hormonal excess in up to 15% of cases, and mild autonomous cortisol secretion in 30–40% of cases. Conclusions: A significant number of TI patients have high LIC, short stature and endocrine disorders. Patients who require occasional transfusions have more liver iron overload and higher hepatic dysfunction. Females appear to attain better final adult height and have higher IGF1- SDS versus males. Our data emphasize the need for long term surveillance for identification of organ-specific risk factors and early disease manifestations.We also recommend a close monitoring of endocrine and other complications, according to the international guidelines.

Conclusions: Low bone mass and deteriorated TBS were noted in approximately two-thirds of T2DM women and was also associated with vertebral fracture. In addition to aging, poor glycemic control may play an important role in bone remodeling, which may be associated with changes in bone strength in T2DM women. Bone strength together with clinical risk factors has the strongest association with fracture, and may potentially be useful to identify women with T2DM at risk.
