Role of 3T MRI in Evaluation of Bone Marrow Changes in Spine in Various Diseases.

Conclusion: Various bone marrow disorders were classified and evaluated separately. A systematic approach to its evaluation by categorization is essential with prudent use of both conventional and problem-solving techniques, such as CSI and DWI, for accurate diagnosis and appropriate patient management. Conventional radiology depicts changes of an altered bony matrix while MRI displays changes at a cellular level and is well suited for imaging the bone marrow. MRI serves as a screening method in bone marrow disorders and the diagnosis is established in context with the clinical findings or by biopsy. PMID: 31304706 [PubMed - in process]

In conclusion, we showed hypermethylation of CpGs as a novel mechanism of action for DNMTi agents and identified 638 hypermethylated molecular targets (CpGs) common to decitabine and azacytidine therapy. These novel results suggest that hypermethylation of CpGs should be considered when predicting the DNMTi responses and side effects in cancer patients. Introduction DNA methyltransferase inhibitors (DNMTi) are widely used as chemical tools for hypomethylating the genome, with an aim to understand the role of DNA methylation in multiple processes (e.g., X-chromosome inactivation and DNA imprinting) and as an anti-ca...

ConclusionHLH is a devastating disease that potentially culminates in multisystem organ failure if not appropriately treated. At the time of clinical suspicion of HLH, approximately 3 days after the patient's admission, his clinical condition had improved on high dose steroids alone, therefore additional immunosuppression was not initiated. The patient might have had EBV that precipitated his HLH, however at the time of diagnosis his viral load was less than 2000 copies/mL and antiviral therapy was not considered necessary. He continues to be in remission, asymptomatic, 9 months after original diagnosis.Figure.DisclosuresN...

Conclusion: This case of DLBCL in an adolescent girl is extraordinary in many ways. DLBCL, especially the non-GCB type, is a rare disorder in pediatric patients. Predominant bone-marrow involvement has been described in only few older patients with non-GCB-type DLBCL and mostly aggressive clinical course. HLH as a presenting feature is rather observed in T- or NK-cell lymphoma than in DLBCL. Finally, central nervous system (CNS) relapse occurs in only about 6% of adults with DLBCL regardless of CNS prophylaxis. Retinal and intraparenchymal CNS involvement without abnormalities in the CSF has been typically described for pr...

Conclusions:We present the largest experience in EMH without associated MPN, in adults. We describe associated conditions and elaborate further on "idiopathic" EMH (n=12), which often represented an incidental discovery during evaluation of unrelated symptoms. None of the patients with idiopathic EMH harbored occult malignancies or subsequently developed MPN or other myeloid malignancies. Our observations do not support undertaking extensive investigations targeting MPN or other malignancies in idiopathic EMH and simple monitoring might be adequate.DisclosuresNo relevant conflicts of interest to declare.

Peripheral nervous system damage from hematologic malignancies is related to neoplastic cells infiltration of peripheral nerves or to monoclonal antibody production cross-reacting with peripheral nerves' antigens. Neurolymphomatosis (NL), a rare manifestation of hematologic malignancies, occurs when malignant cells invade the peripheral nerves leading to various manifestations. Here, we report a case of NL with 2 hematologic malignancies in a 79-year-old woman presenting with lower extremity pain/weakness. Investigation revealed anemia, IgM kappa monoclonal gammopathy, and elevated anti-MAG titer. Electrodiagnostic studies...

AbstractBone marrow examination is integral to the diagnosis of hematological and many non-hematologic disorders. With increase in the elderly population, rise in the incidence of age-related health conditions is being observed. There are few published studies on the disorders involving the bone marrow exclusively in the geriatric age group. We studied the bone marrow profile in the elderly age group (60  years and above) over a period of 1 year using bone marrow aspiration and biopsy. Anemia was the most frequently diagnosed entity followed by multiple myeloma and non-Hodgkin lymphoma. Cases of leukemia, primary...

We describe a 46-year-old female with history of progressive dyspnoea. Physical examination was revealed massive left pleural effusion. Complete blood count revealed anemia, trombositopenia and normal leucocyte count. Viral serology test shown positive of HBsAg and total antiHBc. Chest X-ray revealed left pleural effusion. Pleural fluid cytology was myeloblast consistent with acute myeloid leukemia (AML). Bone marrow aspiration smear, bone marrow biopsy smear, and flow cytometry analysis were consistent with acute myeloid leukemia without maturation (AML M0-FAB classification). Aggressive nature killer (NK)-cell neoplasm includes aggressive NK-cell leukemia (ANKL) and extranodal NK/T-cell lymphoma (ENKTL), nasal type. ANKL is rare and is characterized by a systemic neoplastic proliferation of NK-cells, usually with a leukemic presentation. ENKTL is a predominantly extranodal lymphoma, occurring mainly in the upper aerodigestive tract. Both are aggressive neoplasms strongly associated with Epstein-Barr virus (EBV). Here we report two patients with aggressive NK-cells neoplasms localized in the bone marrow (BM) who presented as prolonged fever, anemia, and thrombocytopenia. Both were treated initia...

Authors: Bhatt VR, Bociek RG, Yuan J, Fu K, Greiner TC, Dave BJ, Rajan SK, Armitage JO Abstract Essential thrombocythemia is well-known to transform to other myeloid disorders, such as leukemia; however, the risk for development of lymphoma is not as well studied. This case report discusses a 76-year-old man with a history of prefibrotic post-essential thrombocythemia myelofibrosis on ruxolitinib, who developed anemia, thrombocytopenia, and leukocytosis with peripheral blasts. Results of a bone marrow biopsy and PET and CT scans revealed stage IV leukemic diffuse large B-cell lymphoma. Several days after cessation ...

Conclusion This analysis provides insight into the demographic and disease characteristics of NDMM patients in a range of clinical practices. Creating solid records of baseline patient disease characteristics using suggested NCCN diagnostic work-up and IMWG criteria provides a foundation for monitoring disease progression and response to treatment. Micro-Abstract In this analysis, we focused on the importance of establishing baseline characteristics in multiple myeloma patients in the Connect MM registry. Variations were observed in the percentage of reported baseline data. Creating solid records of baseline patient disea...
